Client Health Info Form
New clients will be asked to complete this general health questionnaire prior to their first appointment. You can print and complete the form at home, or plan to arrive a few minutes early and fill it out at the time of your appointment.
COVID-19 Procedures
COVID protocols have changed several times based on CDC recommendations and government regulations. Please refer to the reminder email you will receive the day before your appointment to know what precautions are currently in place and what to expect at your appointment. You will also find occasional updates on the blog page of this website.
